What is a railway medical assessment?
A railway medical assessment is a structured health assessment that confirms whether a rail safety worker can safely perform their duties. It is a requirement for anyone seeking unescorted or safety-critical access to the rail corridor, and a valid assessment must be recorded in RIW before that access is granted.
The assessment is not a standard GP consultation. It follows a defined format set by the National Standard for Health Assessment of Rail Safety Workers, and must be conducted by a doctor who is accredited as an Authorised Health Professional by the relevant rail operator.

Railway medical categories
Category 1
Required for workers in the most safety-critical roles, including train drivers and network controllers. The most comprehensive assessment.
Includes: medical history and questionnaire, full physical examination, cardiovascular assessment, ECG, vision and colour perception testing, audiometry, musculoskeletal screen, and drug and alcohol screening.
Category 2
Required for workers with a defined safety function on the rail network. Less extensive than Category 1.
Includes: medical history and questionnaire, physical examination, vision testing, audiometry, and musculoskeletal screen.
Category 3
A baseline health clearance for workers with limited safety responsibilities who require unescorted access to the rail corridor.
Includes: medical history, vision testing, audiometry, and musculoskeletal screen.
